Inspector Comments The cook informed that burgers are sometimes cooked to order. There is no consumer advisory present on the menu. An advisory must be added to the menu as well as a reminder which states which products the advisory applies to in order to ensure the consumer is aware of the added risk. Example language: “Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness.” The person in charge agreed to add this to the menu as soon as possible.
Inspector Comments Tempura chicken was observed held at 115F in the hot holding case. A non-functioning bulb was observed inside the hot case and was the likely the cause of the improper holding temperature. The person in charge immediately increased the temperature in the unit and agreed to use time as a control moving forward so the temperature in the hot case won't be as critical.
Inspector Comments The inside of the ice machine had visible mineral buildup and what appeared to be rust. While mineral deposits may not cause foodborne illness, mold may grow inside ice machines and become undetectable when surrounded by mineral deposits. The person in charge agreed to empty out the ice machine and clean the inside thoroughly right away. The machine will be cleaned more thoroughly moving forward.

Inspector Comments Food items in cold-holding in the bottom shelf of the reach-in cooler case were observed to be above 41F including Hot Pocket sandwiches (46.F) and a Big AZ burger (46F). The rate of bacterial growth and possible toxin production can be greatly reduced when foods are held at temperatures of less than 41F. The person in charge (PIC) discarded all food items on this bottom shelf of the cooler including 10 Hot Pockets, 5 Landshire sandwiches, 3 Pierre sandwiches, and 7 Big AZ sandwiches. The PIC placed a thermometer in the bottom shelf, lowered the temperature of the cooler, and will monitor the temperature to ensure that foods are held at 41F.
