(b) Tustumena, Starboard Side: On or about June 13, 2017 and at times prior thereto, top rope rails used for guard rails measured 41 inches and deflected from 39 to 58 inches. Mid-rope rail did not exist. This condition exposed employees to a fall hazard of 26 feet.
