M/V American No. 1, Light Cargo Dock: On or about March 9, 2017 and at times prior thereto, employee was operating a forklift without the use of seat belt protection. This condition exposed the employee to a struck-by hazard in the forklift tipped over. Among other methods, a means of correcting this hazard would be to ensure that employees utilize the installed a safety belts when operating the forklifts.
