Inspector Comments 5 dead rodents were found on 3 sticky traps in the back storage room. Rodent presence has been marked in previous inspections and traps are laid throughout the facility, however the rodent presence is still ongoing. Operator is acquiring both trapping traps and sticky traps from a pest control company. Rodents in the facility pose an adulteration risk to food and food contact surfaces. Operator will utilize a risk control plan to address the rodent issue to get the issue under control to include monitoring, monitoring frequency, methods of trapping and monitoring, cleaning and reporting and progress will be monitored in follow up inspections.
Inspector Comments Chlorine sanitizing solution was below 50ppm on the test strips from operator and EHO. Food that comes into contact with surfaces that are not properly cleaned and sanitized are liable to contamination. The person in charge (PIC) created a new chlorine sanitizing solution that was verified to be at 50 ppm using test strips. The PIC will ensure that all food employees are trained to create a safe and adequate chlorine sanitizing solution: 1 teaspoon of chlorine to 1 gallon of water for a chlorine sanitizing solution between 50-100 ppm. The PIC will also ensure that test strips are available to verify the concentration of chlorine sanitizing solutions.
Inspector Comments At time of inspection, food worker cards were not able to be shown and verified by inspectors. A copy is required to be kept on file for all employees that handle food. Please visit http://dec.alaska.gov/eh/fss/Training.html to obtain training and to take the Food Worker Card test for any employees that do not have a current food worker card. Person in Charge will keep copies of food worker cards on file to present when requested by officials. Copies of the FWCs will be emailed to the department.
Inspector Comments A copy of the certificate for each Certified Food Protection Manager (CFPM) employed was not available at the time of inspection. The manager states he has a current one and will forward a copy to the department. A person who is immediately available and knowledgeable in operational and Code requirements is needed to establish an important barrier to food contamination. Inspector Comments Sticky traps placed throughout the restaurant were observed to have trapped rodents (photo attached). A total of 12 dead rodents were observed caught in sticky traps in the restaurant in areas such as the walk-in cooler, under the make line cooler, in the host's station, and in the dry storage area. These sticky traps must be routinely inspected and removed when a rodent is observed caught. Decaying rodents in the kitchen could be a significant health hazard to anyone ingesting food prepared in the facility. The sticky traps with caught rodents were discarded immediately and replaced with new ones. The person in charge agreed to check these traps daily and replace them when necessary moving forward. An increase in the number of traps placed throughout the facility will be required to diminish the population in the restaurant as well. Additionally, there was a significant amount of food debris under the make line cooler which could attract rodents (photo attached). This area needs to be swept daily to not leave out a food source for rodents.

Inspector Comments Three separate food contact surfaces were observed to not be properly cleaned: the underside of the large cutting board in the back of the kitchen, the point of the can opener, and the inside of the ice machine. It is critical to wash, rinse, and sanitize food contact surfaces thoroughly to prevent the growth of pathogenic bacteria which can lead to foodborne illness. The person in charge agreed to have the ice machine cleaned out and scrubbed thoroughly, the cutting board washed rinsed and sanitized, and the can opener washed, rinsed, and sanitized. Staff will be reminded of the importance of cleaning these and other surfaces thoroughly moving forward.
Inspector Comments The back door in the kitchen has visible gaps where rodents can easily enter. The presence of pests in a food service facility can lead to contamination of food and foodborne illness. Numerous apparent mouse droppings were observed in the facility, mostly inside the storage room. The person in charge agreed to have the gaps on the door sealed within two weeks.

Inspector Comments Chicken was observed at 47F after cooling at least overnight. To avoid pathogenic bacterial growth foods must be cooled from 135-70F in two hours and 70-41F in an additional four hours. Cooked chicken was voluntarily discarded and person in charge will adopt a new cooling process for cooked foods immediately.
Inspector Comments Hood is covered with debris which could potentially fall into food. Hood should be cleaned thoroughly and kept clean to avoid possible contamination of foods.
