Yard shop; on or about 15 July 2015 and times prior thereto; the work rest on a Baldor bench grinder, serial number 1184, was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured 1/4 of an inch. This condition exposed employees to a caught in hazard. Piston and Rudder, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1915.134(b), which was contained in OSHA inspection number 419043, citation number 1, item number 4 and was affirmed as a final order on 11 July 2012, with respect to a workplace located at the Machine/fabrication shop. |
