Yard shop; on or about 15 July 2015 and times prior thereto; the work rest on a Baldor bench grinder, serial number 1184, was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel. The gap between the abrasive wheel and work rest measured 1/4 of an inch. This condition exposed employees to a caught in hazard. Piston and Rudder, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1915.134(b), which was contained in OSHA inspection number 419043, citation number 1, item number 4 and was affirmed as a final order on 11 July 2012, with respect to a workplace located at the Machine/fabrication shop. |

29 CFR 1915.507(b)(1): The employer must select, install, inspect, maintain, and test all portable fire extinguishers according to NFPA 10-2002 Standard for Portable Fire Extinguishers (incorporated by reference, see 1915.5). a) On or about May 1, 2012 and times prior thereto, none of the fire extinguishers in the facility had monthly inspections completed. This exposed employees to burns and smoke inhalation from the possibilty of an fire extinguisher not being servicable. b) Machine shop: On or about May 1, 2012 and times prior thereto, a fire extinguisher was blocked. This exposed employees to smoke inhalation hazards from delays in accessing the fire extinguisher. |
