Citation text line 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Main Maintenance Shop; on or about 23 March 2016 and times prior thereto; three instances were noted where electrical cords were damaged: a. Bays 6 and 7; a portable light stand had damaged to the outer insulation where the electrical cord and plug attach exposing wires. b. Wash Bay; a 50 foot blue extension cord had damaged to the outer insulation where the cord and plug attach exposing wires. c. Bay 9; the electrical cord on the oil filter crusher had damage to the outer insulation where the cord and plug was damaged. This condition exposed employees to electrical shock and electrical burn hazards.
