(a) Kloosterboer Warehouse - On or about March 5, 2020 and at times prior thereto, employees were exposed to being caught in the pinch and nip points of the conveyor system in the Kloosterboer warehouse while offloading the processed fish boxes from F/V Phoenix. In the event and employee were to get caught in the pinch and nip points of an unprotected conveyor system is likely to cause severe lacerations and/or amputation.

Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Cold Storage Facility, North Side: On or about February 11, 2015 and at times prior thereto, an employee operating a Genie Model Z-45/25, Model 468-45-5098, Self-Propelled Articulating Boom Lift through the terminal area and raising lift up to 20 feet without utilizing a positioning belt/harness and lanyard. This condition exposed the employee to a fall hazard of approximately 20 feet in the event of equipment failure. Kloosterboer Dutch Harbor, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, OSH ACT of 1970 Section (5)(a)(1), which was contained in OSHA inspection number 958957, citation number 1, item number 1 and was affirmed as a final order on 27 March 2014, with respect to a workplace located at Ballyhoo Road, Dutch Harbor.

Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Cold Storage Facility, North Side: On or about February 11, 2014 and at times prior thereto, an employee operating a Genie Model Z-45/25 Self-Propelled Articulating Boom Lift 32 feet did not utilize a positioning belt/harness and lanyard. This condition exposed the employee to a fall hazard of approximately 32 feet in the event of equipment failure. Among other methods, one feasible and useful abatement method would be to provide employees with a positioning belt or harness and positoning lanyard which is secured to the personnel basket according to manufacturer's requirements.
