Shop bay one Mezzanine; on or about 08 May 2019 and times prior thereto; the employer did not ensure that unguarded edges were effectively protected. An unguarded edge measuring approximately 126 inches above the lower surface did not have guardrails installed. This condition exposed employees to a fall hazard. |
