The Notice tag located in the motor room for the air compressor # 3; was not marked identifying the individual that applied the tag. This condition exposed employees to inadvertent start-up of the equipment resulting in caught-in hazards.

This condition exposed employees to electrical shock and inadvertent start-up when servicing the downstream equipment. (b) Laundry/Compressor Room: On or about May 20, 2013 and at times prior thereto, the Eaton/cutler-Hammer disconnect for the Jefferson Electric 240-480 volt transformer was not legibly marked. This condition exposed employees to electrical shock when servicing the downstream equipment.

Oil spill measured at approximately 36 inches in the aisle way. This condition exposed employees to slipping and fall hazards when working or transiting through the Oil Separator Test Room.

This exposed employees to smoke inhalation and/or fire hazards from delays in exiting the building.
